GOAL MAMA PRIVACY POLICY

Welcome to Goal Mama. The Goal Mama App (the “App”) is designed for Nurse-Family Partnership mothers and their nurse home visitors to track certain activity during the mother’s pregnancy and first two years of motherhood. All App users must be at least thirteen (13) years of age.

Several organizations are involved in collecting and analyzing data (including health information and other personal information) collected through the App, including Ayogo (the company that built and operates the App), Nurse-Family Partnership National Service Office (“Nurse-Family Partnership”), the nonprofit that helped create the App and has authorized its use by Nurse-Family Partnership-trained nurses), your nurse home visitor and his/her employer, and Hopelab Foundation Inc. (“Hopelab”), the nonprofit that helped design the App and is conducting the Study.

This privacy policy (“Privacy Policy”) is designed to answer your questions regarding the information the App will collect about you and who it will be shared with. In this Privacy Policy, when we say “we” or “us” or “our” we are referring to Ayogo, and when we say the “Companies,” we are referring to Ayogo, Nurse-Family Partnership and Hopelab.

WHY WOULD YOU WANT TO CREATE AN ACCOUNT?

In order to use the App, get information from the App, and get the benefits of the features of the App, you have to first create an account.

HOW DO YOU GET STARTED?

To sign up and create an account, you’ll be asked for information such as your email address, your first and last name, your date of birth, your baby’s due date, and the name and gender of your child (if your child has already been born). We will also ask you to create a username and a password.

ONCE YOU HAVE CREATED AN ACCOUNT, WHAT INFORMATION WILL WE COLLECT ABOUT YOU?

Once you have an account, we will collect and store the following information about you (“Personal Information”):

We will also collect information you choose to provide through the App, potentially with the guidance of your nurse home visitor, such as information and notes related to your nurse home visits, aspirational statements, goals, challenge and quiz answers, and information that you provide in the Goal Mama community forum (“Community Forum”) and other areas of the App where you can enter information. You should consider whether or not you would like to share the information that you enter into the App. Furthermore, any information provided in the Community Forum will be visible to other users, so you should not share in the Community Forum any personal information, such as your full name, address, telephone number or other details that can be used to identify you, or other information that you do not want to make public. Please review the Goal Mama App Community Guidelines before deciding whether to post information in the Community Forum. IS THERE ANY INFORMATION THAT WE AUTOMATICALLY COLLECT ABOUT YOU?

We may use cookies and other technologies to recognize you as you use or return to the App so that we can understand how the App is used. These technologies collect certain information automatically, such as:

We will collect such information once per month.

We may collect the physical location of your mobile device by using, for example, satellite, cell phone tower, GPS or WiFi signals. We use your device’s physical location to provide you with location-based services and content. You may be able to allow or deny such collection and/or use of location data through your device settings.

We may use third-party services in connection with the App. Such services may use cookies and/or similar technologies to collect and analyze information about App use and to report on activities and trends. Such services may also collect information regarding the use of other websites, apps and online services.

If we combine the information described in this section with information that is deemed Personal Information, we will treat the combined information as Personal Information for as long as it is combined.

HOW DO THE COMPANIES USE YOUR INFORMATION?

We may use your Personal Information to administer the App, including sending you messages about your account or your use of the App, and to improve the App and related services. If you use the Community Forum within the App, we may view and store your activity to ensure its compliance with the Goal Mama App Community Guidelines.

Nurse-Family Partnership and Hopelab may use your Personal Information for analysis and evaluation related to your participation in the Nurse-Family Partnership program.

In addition, the Companies or any of them may use de-identified user data for research, improving products and services, analyzing, tracking and reporting on user engagement, publishing information regarding the efficacy of the App and related services, and for any other similar purpose.

WHO DO THE COMPANIES SHARE YOUR INFORMATION WITH?

For the purposes described above, Ayogo may share your Personal Information with Nurse Family Partnership, Nurse-Family Partnership-trained nurses, including your nurse home visitor and his/her employer, and Hopelab. The Companies may also share your Personal Information among themselves for analysis and evaluation related to the Study.

COULD YOU ACCIDENTALLY REVEAL MORE INFORMATION ABOUT YOURSELF?

As described above, any information posted in the Community Forum will be visible to other users, so we encourage you not to disclose in the Community Forum any personal information, such as your full name, address, telephone number or other details that can be used to identify you, or other information that you do not want to make public. The moderators may remove such information in their discretion. We are looking out for you, but you should look out for yourself as well. It is important for your safety to protect any information about yourself that you do not want anyone to know.

Also, please note that any push notifications you receive may appear on your mobile device as soon as they are received, even when the device is locked. These messages could therefore be seen and read by others who are near your mobile device when the messages are received.

WHAT ARE YOUR PRIVACY CONTROLS?

You may stop all collection of information by the App by uninstalling the App. If applicable, you may stop the receipt of push notifications through your device settings.

If you uninstall the App, we will not ask for any new information about you, but we will use whatever information you have already shared in accordance with the Informed Consent you signed before beginning participation.

WHAT ELSE DO YOU NEED TO KNOW ABOUT PRIVACY?

CHILDREN’S PRIVACY

The App is not directed to individuals under the age of thirteen (13), and we do not knowingly collect personal information from such individuals. If you are at least thirteen (13) years of age but less than eighteen (18) years of age, you will need your parent’s or legal guardian’s permission to use the App. Please consult with your parent or legal guardian before installing the App.

DO NOT TRACK

We comply with the California Do Not Track legislation of 2014 and make the following disclosure: because the Do Not Track signal we receive does not often reflect a user's privacy preferences, our App does not react to the Do Not Track signal.

THIRD-PARTY SERVICES

This Privacy Policy does not address, and we are not responsible for, the information collection, use, disclosure or security practices, or other practices, of any third party, including any third party operating a service to which the App links. The inclusion of a link within the App does not imply our endorsement of the linked service.

CROSS-BORDER TRANSFER

The App is intended for use only by individuals in the United States. The information we collect through the App may be stored and processed in any country where we have facilities or in which we engage service providers, and by using the App, you consent to the transfer of information to countries outside of your country of residence, including the United States, which may have different data protection rules than those of your country.

WILL THIS POLICY EVER CHANGE?

We may need to update the Privacy Policy as we add or modify features of the App or if there are changes in the law. We may also need to update the Privacy Policy if we decide to collect more information about you. If we make a small change to the Privacy Policy, we probably will not notify you, but we will change the “effective date” at the top of the Privacy Policy. If we make changes to the Privacy Policy that will materially affect you, we will tell you by sending an email to the email address currently listed for you. You should expect to receive an email from us if we materially change the way we collect, use and/or share your Personal Information.
